About Horizon NJ Health
Horizon NJ Health is New Jersey's largest Medicaid managed care plan, a subsidiary of Horizon Blue Cross Blue Shield of New Jersey. It covers more than 854,000 members enrolled in NJ FamilyCare (Medicaid) and provides access to over 67,000 doctors, specialists, and health professionals statewide. Horizon NJ Health was the first Medicaid managed care plan offered in all 21 New Jersey counties.
Who is eligible?
New Jersey families who qualify for NJ FamilyCare (Medicaid) based on household income. Children from birth through age 18 are eligible if the family meets income guidelines. Pregnant women, parents/caretakers, and children in foster care may also qualify. Apply through NJ FamilyCare at njfamilycare.dhs.state.nj.us or by calling 1-800-701-0710.
