How to join
Already have a child? Switch in three steps.
- 1Pick the closer office. Book a checkup online, or call us to schedule another type of first visit.
- 2Request your child's records from your previous pediatrician and have them sent to us. Our team can help walk you through it.
- 3Bring your insurance card and any school or camp forms. We'll take it from there.
Baby on the way? Tell us early.
- 1Fill out the expecting parent form below. We'll keep your info on file and reach out before your due date.
- 2When baby arrives, call us within 48 hours and we'll book your first visit.
- 3Our team will help arrange the right first visit. Bring the discharge paperwork. That's it.
