How to join
Already have a child? Switch in three steps.
- 1Pick the closer office and book your first visit.
- 2Request your child's records from your previous pediatrician and have them sent to us. Our team can help walk you through it. Here's how.
- 3Bring your insurance card and any school or camp forms. We'll take it from there.
Baby on the way? Tell us early.
- 1Fill out the expecting parent form below. We'll keep your info on file and reach out before your due date.
- 2When baby arrives, call us within 48 hours and we'll book your first visit.
- 3First visit happens 2-3 days after you're home. Bring the discharge paperwork. That's it.

Fill out the form and we'll be in touch before baby arrives. A few things you can expect:
- An email confirming we received your registration.
- A note from our team a few weeks before your due date.
- A first visit scheduled 2-3 days after you're home from the hospital.
